Job Summary:
The nurse is responsible for monitoring and managing the health and medical needs of individuals with special needs, including intellectual and developmental disabilities (IDD) and other chronic conditions. The nurse works collaboratively with caregivers, physicians, and interdisciplinary teams to ensure comprehensive care and improve the quality of life for these individuals.

Key Responsibilities:

1. Health Monitoring and Assessment:

  • Conduct routine physical health assessments and screenings.
  • Monitor vital signs, including blood pressure, heart rate, weight, and other health indicators.
  • Identify any changes in health status and communicate findings to the healthcare team.
  • Administer medications as prescribed and monitor for side effects or adverse reactions.
  • Manage chronic conditions such as diabetes, epilepsy, or respiratory issues through regular monitoring.

2. Education and Advocacy:

  • Educate individuals, caregivers, and families about health conditions, treatments, and preventive care.

3. Documentation and Reporting

  • Prepare reports for care teams and regulatory agencies.

4. Health Promotion and Preventive Care:

  • Promote healthy lifestyles through nutrition counseling, exercise recommendations, and hygiene education.

Qualifications:

  • Licensed Practical Nurse (LPN) or Registered Nurse (RN) with current state licensure.
  • Experience in working with individuals with intellectual and developmental disabilities is highly preferred.
  • Strong knowledge of chronic conditions and special needs healthcare.
  • Excellent communication and organizational skills.
  • Ability to work collaboratively with a multidisciplinary team.

Work Environment:

  • May work in group homes, residential facilities, or community health settings.
  • Role requires both clinical knowledge and compassionate care tailored to the unique needs of individuals with disabilities.
